Please read all copy below BEFORE filling out the Appointment Request Form.
A major credit card or debit card is required when making your 1st appointment. It is not required for any future appointments.
If you're a NO SHOW (meaning you don't show up for your scheduled waxing appointment even if the reason is because you didn't get an appointment reminder), your credit or debit card will still be charged 100% of the total treatment(s).
If you need to cancel or reschedule your appointment, a 24-hour notice is required for a single service and a 48-hour notice is required for two (2) or more services otherwise you will be charged 50% of the treatment(s).

Please understand that it is difficult to re-book same day cancellations and those cancellations the night before your morning appointment. There are exceptions to the cancellation policies in which you would not be charged which are:
- If you are ACTIVE MILITARY and are called into work
- If you are legitimately ill (please note menstrual cycle issues do not apply)
- If you're a single parent and your minor child who lives with you is ill
- If you have Covid-19 or someone you live with has Covid-19
PLEASE NOTE: If you are a civilian and called into work but CHOOSE to cancel your waxing appointment without giving a 24 or 48-hour notice, you will be charged 50% of your total treatment(s).
As a courtesy to our clients, you may receive an appointment reminder via text. Please understand you're still responsible for showing up and on time to your appointment even if you don't receive a courtesy text.

If you're a NEW CLIENT and do not fill out this form completely (i.e. you don't include your credit card info) then your appointment will not be processed.
